Medical Information From The Nurse
Please call the school office (988-0679) before 7:30AM the morning of the absence. If it is to be a prolonged absence, please notify the school counselor-Mrs. Pierce. A pupil returning from an absence must have a dated and signed note from the parent explaining the reason for the absence. Students who are out 5 or more days must have a physician"s approval to return to school. Those with communicable diseases must present to the school nurse, Mrs. Mathew, written evidence of being free of communicable diseases. All medication to be taken during school hours must be left in the Health Office with Mrs. Mathew. They must be accompanied by a note from both the doctor and the parent. This includes even over the counter medications. Physicians may fax the note to Jaggard at 988-7788. Parents should set this up with the doctor. No medication will be administered without these notes. Medication must be in its original container or pharmacy-labeled container. Last Modified on October 20, 2008
