Dear Parent/Guardian:

The Spring Sports Season, including baseball, girl’s softball, girl’s lacrosse, boy’s lacrosse, and track will begin in late February and March.  Any student who wishes to try out for a sport must have a physical exam on file in the nurse’s office. The physical must have been completed within the past year (365 days). If the physical was done at the time of a previous sport season, a Seasonal Sport Update form must be signed by a parent and returned to the nurse before tryouts.

Students cannot attend tryouts unless their paperwork is complete.

If obtaining a physical poses a financial hardship for you, please contact the school nurse as soon as possible to make alternative arrangements.

Please note the following important information:

·         All physicals must be completed using the forms required by New Jersey, and provided by the school. NO OTHER FORMS WILL BE ACCEPTED. These are in the nurse’s office or on the DeMasi website at www.evesham.k12.nj.us/dms. Click on “Athletics”.  Please note that the entire form, including the history, must be reviewed by the medical provider at the time of the physical.

·         Physical exams may not be done by a chiropractor.

·         Sports physicals must have been completed within 365 days of the first day of tryouts for any given sport.

·         All physicals must be reviewed by the school physician, per regulation. Therefore, they must be submitted by the deadline.

·         If your child might need emergency medication during after school activities, either an inhaler or Epipen, a self-medication form must be completed by your physician. Forms are available from the nurse.

PLEASE CHECK YOUR FORMS BEFORE LEAVING THE DOCTOR’S OFFICE TO MAKE SURE THEY ARE COMPLETE: Incomplete forms will be returned and student may not try out.

ALL REQUIRED FORMS MUST BE SUBMITTED TO THE SCHOOL NURSE BY:

FRIDAY, FEBRUARY 22^nd for GIRLS SOFTBALL ONLY

FRIDAY, MARCH 2nd for ALL OTHER SPORTS
